The Discover Home Program provides nephrology fellows with the educational tools to evaluate current and evolving systems of dialysis patient care, appraise and assimilate scientific evidence about HHD, and pursue quality improvement for dialysis patients.

Every patient deserves to know all their options for dialysis. As physicians, it’s our job to make that a reality.
A “Home First” philosophy has been identified by nephrology leaders as an important goal for providers and KRT patients in the United States.
While the number of patients with kidney disease is steadily increasing in the United States, home dialysis therapy, particularly home hemodialysis (HHD), is underused as a modality for the treatment of chronic kidney failure.
Our mission is to bring more patients home by minimizing barriers to starting and successfully continuing HHD therapy.
Only 2% of dialysis patients are currently on HHD
90% of physicians would choose home modalities for themselves1
67% of fellows feel minimally prepared or not prepared at all for HHD2
