Nurse
The Discover Home Program provides nephrology nurses with the educational tools to evaluate current and evolving systems of dialysis patient care, appraise and assimilate scientific evidence about HHD, and pursue quality improvement for dialysis patients.
The Discover Home Program includes:
Interactive course modules about HHD and vascular access, led by expert nephrology nurses
Resources and toolkits for learners who would like to expand their research
Teaching tools for nurse educators and administrators
The future of dialysis is home.
Every patient deserves to understand their options for dialysis, from types of access to modalities.
As nurses, committed to the highest quality of patient care, it’s our job to make that a reality.
A “Home First” philosophy has been identified by nephrology leaders as an important goal for providers and KRT patients in the United States.
While the number of patients with kidney disease is steadily increasing in the United States, home dialysis therapy, particularly home hemodialysis (HHD), is underused as a modality for the treatment of chronic kidney failure.
Our mission is to bring more patients home by minimizing barriers to starting and successfully continuing HHD therapy.
89% of nephrology nurses would choose home modalities for themselves1
Only 2% of dialysis patients are currently on HHD2
70% of patients will choose a home modality if provided with kidney disease education3
